Patient Registration

How do I register with the practice?
Please bring along your medical car which the receptionist can help you complete. If you do not possess a medical card ask at Reception for an application form (GMC1). If you would like to meet the doctor before registering please make an appointment.

New patients will be asked to complete a personal health questionnaire and to make an appointment with the Practice Nurse for a basic health check. If you have a complicated medical history or take regular medication please make an additional appointment with the doctor after your basic Health Check.

Visitors/Holiday makers are registered on a special form (T/R) provided at Reception.

You will be registering with the Practice and can therefore see any of the doctors in the Practice. However, it is preferable to remain with the same doctor for the duration of treatment of a particular illness and you may select a “preferred doctor” when you register.

Pre-registration

If you wish to pre-register click on the link below to open the form. When you have completed all of the details, click on the 'Send' button to mail your form to us. When you visit the surgery for the first time you will be asked to sign the form to confirm that the details are correct.
