How do I register with the practice?

register_nowPlease bring along your medical card which the receptionist can help you complete. If you do not possess a medical card ask at Reception for an application form (GMC1). If you would like to meet the doctor before registering please make an appointment.

New patients will be asked to complete a personal health questionnaire and to make an appointment with the Practice Nurse for a basic health check. If you have a complicated medical history or take regular medication please make an additional appointment with the doctor after your basic Health Check. Please bring two forms of ID with you, one of which must be a recent utility bill.

Visitors/Holiday makers are registered on a special form (T/R) provided at Reception.

You will be registering with the Practice and can therefore see any of the doctors in the Practice. However, it is preferable to remain with the same doctor for the duration of treatment of a particular illness and you may select a “preferred doctor” when you register.
